Next Gen NCLEX case study: What You Need to Know

Looking for help with Next Gen NCLEX case study? You are in the right place. This guide gives nursing candidates across the USA and Canada clear, practical steps to prepare for the NCLEX with confidence.

Practice only: This is an original case study written by Dr. Blake NCLEX Review to teach clinical judgment. It is not an actual NCLEX exam item and is not affiliated with NCSBN.

How Next Gen NCLEX Case Studies Work

Each NGN case study follows one client and asks six questions that move through the Clinical Judgment Measurement Model: recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action and evaluate outcomes. Read the scenario, then answer all six questions and check your rationale.

Client Scenario

0800 – Nurses’ Note: A 68-year-old client is admitted with a 2-day history of productive cough, fever and increasing shortness of breath. The client reports feeling weak and “can’t catch my breath.”

Vital Signs: Temp 38.9°C (102°F) · HR 112 · RR 26 · BP 104/62 · SpO2 88% on room air.

Assessment: Crackles in the right lower lobe, productive cough with yellow-green sputum, oriented but restless, skin warm and flushed.

Question 1
Select all that apply

Recognize cues. Which findings require immediate follow-up? Select all that apply.

Correct answer: A, B, D
The low oxygen saturation, high fever and elevated respiratory rate are abnormal and need prompt action. Being oriented is a normal finding; the blood pressure is borderline but not the priority cue here.
Question 2
Choose one answer

Analyze cues. The findings are most consistent with which condition?

Correct answer: B
Fever, productive yellow-green sputum, localized crackles and hypoxia point to pneumonia (an infectious process).
Question 3
Choose one answer

Prioritize hypotheses. The nurse’s priority problem for this client is:

Correct answer: A
With SpO2 88% and respiratory distress, impaired gas exchange (an airway/breathing priority) is the top concern.
Question 4
Select all that apply

Generate solutions. Which interventions are appropriate at this time? Select all that apply.

Correct answer: A, B, C
Oxygen, an upright position and obtaining cultures before antibiotics all support gas exchange and treatment. Lying flat worsens breathing, and fluids are usually encouraged unless contraindicated.
Question 5
Choose one answer

Take action. After applying oxygen, which action should the nurse do next?

Correct answer: A
Once cultures are obtained, timely antibiotic administration treats the underlying infection. Removing oxygen or delaying care would harm the client.
Question 6
Choose one answer

Evaluate outcomes. Two hours later, which finding indicates the interventions were effective?

Correct answer: A
Improved oxygen saturation and a normalizing respiratory rate show gas exchange is improving. The other options indicate worsening status.

Clinical Judgment Study Tips

  • Read the entire scenario and every data tab before answering.
  • Ask: what is the most urgent cue, and what problem does it point to?
  • Match your actions and monitoring to the identified problem.
  • Re-read the case when a later question adds new data.
